Working on a recent site for a school I needed a new approach to handling page heirachy. I’ve quite often concluded that two levels is enough for a clean menu system. That takes care of parents and children, but what about grandchildren? I’ve sometimes listed them in a side bar on the child pages. See this post: /show-grandchildsubpages/. On other occasions it has seemed appropriate to show the grandchild page content on its parent; the child page.  See this post: /show-sub-page-content-on-page/. On the new site I combined these two approaches using a full width template to show grandchildren below the parent content and a sidebar template to list the grandchildren in the sidebar.
